Qualifications

• One year of case management experience is required.
• Case Management Certificate preferred.
• Bilingual Spanish/English required.

Responsibilities

• Conduct telephonic and/or face-to-face assessments and develop care plans.
• Manage a caseload of supportive and medically complex members.
• Coordinate services with PCPs and specialty providers.
• Provide condition management education and medication reviews.
• Coordinate with members to connect to community resources and support services.
• Monitor progress and document care coordination activities.
• Escalate quality of care issues through established channels.
• Utilize negotiation and motivational interviewing to engage members.
