Job summary

Talentify is seeking a Case Management Coordinator in Bradenton, FL to coordinate case management for Medicaid Long Term Care enrollees and support care planning, education, and care coordination.

Ideal candidates have at least one year of case management experience, a Case Management Certificate, and bilingual Spanish/English; a Bachelor's degree in social work or related field is preferred; proficiency with Microsoft Office is expected.

Qualifications

Responsibilities

• Coordinate case management activities for Medicaid Long Term Care enrollees.
• Collaborate and inform the case management process to promote optimal health outcomes.
• Evaluate members using care management tools and data review.
• Coordinate care plan activities and monitor progress.
• Identify and escalate quality of care issues through established channels.
• Employ negotiation to meet members' benefits and healthcare needs.
• Engage members through motivational interviewing to support healthy lifestyle changes.
• Provide coaching and support to empower informed healthcare decisions.
• Assist members in participating with providers in decision-making.
• Ensure compliance with regulatory and accreditation guidelines.
