Job summary

ICONMA seeks a bilingual Medicaid Care Manager to join our remote team in Florida. The role focuses on coordinating case management for Long Term Care and Comprehensive Program enrollees, using evidence-based tools to support members.

Responsibilities include care plan coordination, information and education for members, collaboration with providers, and ensuring compliance with regulatory standards. Fluent Spanish/English required; case management experience preferred.

Qualifications

• Fluent in Spanish/English (reading, writing, speaking).
• One year Case management experience required; Case Management Certificate preferred.
• Long term care experience preferred.
• Microsoft Office; strong in Excel.
• Bachelor's degree in Social Work or related field preferred.

Responsibilities

• Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.
• Uses critical thinking and judgment to inform the case management process for optimal healthcare outcomes.
• Conducts comprehensive evaluation of members using care management tools and data review.
• Coordinates and implements care plan activities and monitors progress.
• Performs multidisciplinary reviews to achieve optimal outcomes.
• Identifies and escalates quality of care issues through established channels.
• Utilizes negotiation to secure options and services meeting member needs.
• Employs motivational interviewing to maximize member engagement and health changes.
• Provides coaching and support to empower members to make informed choices.
• Helps members participate with providers in healthcare decision-making.
• Monitors, evaluates and documents care; adheres to regulatory guidelines.
