The role covers intake, screening, assessment, referral, linkage, and follow‑up, with participation in community events and outreach efforts. Requires a Bachelor’s degree and bilingual English/Spanish.

Qualifications

• Bachelor’s degree in Social Work or a Human Services field or equivalent experience.
• Two years’ experience providing information and referral services.
• One or more years in crisis counseling or clinical work preferred.
• Strong interpersonal skills and calm communication in emergencies.
• Bilingual in English/Spanish.

Responsibilities

• Provide follow‑up services, advocacy, and care coordination for clients.
• Assist with intake, screening, needs assessment, referral, and linkage.
• Conduct person‑centred needs and service assessments; address safety or crisis concerns.
• Facilitate referrals to treatment or service providers or facilities.
• Advocate to access federally funded and private services for basic needs.
• Prepare correspondence, case notes, reports, and related documents using computers.
• Maintain collaborative relationships with community agencies and partners.
• Represent the Contact Center at outreach events as a community liaison.
