Job summary

Wider Circle is hiring a Care Advocate to join the care delivery team in Texas. You will support the WiderCare+ program, bridge clinical care with social needs, and drive CHI and PIN initiatives to balance medical and social support for high-risk patients.

You will conduct SDOH assessments, coordinate with clinicians, manage workflows, and document in the CMS system. A CHW Texas certification and bilingual English/Spanish skills are required, with 3+ years in community outreach or healthcare.

Qualifications

• Active CHW certification in Texas required.
• Bilingual in English and Spanish required.
• 3+ years in community outreach or healthcare.
• Strong digital literacy and ability to use web/app tools.
• Reliable transportation and valid driver's license.

Responsibilities

• Execute PIN care plans and monitor disease-specific plans.
• Coordinate care between patients and providers and support post-discharge follow-up.
• Conduct SDOH assessments and develop action plans for social needs.
• Help patients navigate Texas healthcare systems and arrange transportation or telehealth access.
• Update CMS with patient interactions and ensure documentation accuracy.
• Refer patients to community resources and establish local partnerships.
• Provide behavioral support and motivational interviewing to improve adherence.
• Maintain CMS compliance and detailed activity logs.
