We are seeking a compassionate, organized, and culturallyresponsive Care Coordinator / Community ResourceSpecialist to support clients throughout their treatmentjourney. This position serves as a vital link between clients,families, healthcare providers, and community resources, ensuringindividuals receive the services and support necessary to achievepositive outcomes.

The successful candidate will engage clients from referral throughtreatment, coordinate care and services, assist with benefits andinsurance enrollment, and provide resource navigation and advocacy.This role requires strong relationship-building skills, attention todetail, and a commitment to serving diverse populations.

Key Responsibilities
  • Engage clients from the point of referral through successfulcompletion of treatment services.
  • Coordinate care and support services to address client needs andtreatment goals.
  • Assist clients with benefits eligibility determination andenrollment in public and private insurance programs.
  • Conduct social needs screenings and identify barriers to care.
  • Provide resource referrals and service navigation assistance.
  • Support clients and families in accessing clinical, behavioralhealth, and community-based services.
  • Develop and maintain strong relationships with communityagencies, referral sources, and service providers.
  • Facilitate community resource linkage and coordinate appropriate referrals.
  • Maintain accurate client records, service data, and required reports.
  • Ensure compliance with HIPAA regulations, Joint Commissionstandards, agency policies, and documentation requirements.
  • Participate in interdisciplinary team meetings and collaborativecare planning.
  • Contribute to quality improvement initiatives and programdevelopment efforts.
  • Provide ongoing support, education, and advocacy for clients andtheir families.
  • Perform other duties as assigned.
Experience and Skills
Education
  • Bachelor's degree in Social Work, HumanServices, or a related field preferred.
  • Candidates with an Associate's degree andrelevant human services experience will be considered.
Experience
  • Minimum of three (3) years of directexperience in Substance Use Disorder (SUD) treatment.
  • Knowledge of case management protocols andevidence-based practices.
  • Experience working with diverse andunderserved populations.
  • Demonstrated cultural competence andcommitment to equitable service delivery.
  • Strong verbal and written communication skillswith the ability to effectively engage clients,families, healthcare providers, and community partners.
  • Ability to work independently while managingmultiple priorities.
  • Experience maintaining accurate records and documentation.
  • Proficiency in Microsoft Office applications,including Word and Excel, and general computerskills in a Windows environment.
