Job summary

HeartShare St.Vincents seeks a Care Manager in the Integrated Health Department to coordinate care for clients with complex medical and psychiatric needs. You will liaise with PCPs, psychiatrists, therapists, and community partners to ensure comprehensive access to services.

Associates degree preferred; two years’ case management experience required. Travel across all five boroughs for home visits is expected, with mileage reimbursement available.

Qualifications

• Minimum of two years’ experience in case management or related medical field.
• Ability to travel across all 5 boroughs for home visits.
• Fluent in Russian or Spanish and English.

Responsibilities

• Coordinate care team activities for clients with complex conditions.
• Outreach to clients to facilitate keeping scheduled appointments.
• Assist with care plans and discharge planning with providers and families.
• Review benefits, entitlements, and housing with clients and assist applications.
