Home Family Care in Elmhurst, Queens is seeking English/Spanish Care Coordinators to coordinate and manage home care services, assess client needs, develop care plans, schedule caregivers, and monitor service delivery. The role emphasizes accurate documentation and clear communication with clients, families, and healthcare professionals.

We offer a 9am-5pm schedule and a full-time position with opportunities to collaborate with a multidisciplinary team in a home-based setting.

Qualifications

We are looking for both English/Spanish Care Coordinators

Job Summary: The Home Care Coordinator is responsible for coordinating and managing home care services to ensure clients receive high-quality, compassionate, and individualized care. This role involves assessing client needs, developing care plans, scheduling caregivers, monitoring service delivery, maintaining accurate documentation, and communicating with clients, families, and healthcare professionals.

Key Responsibilities

• We have our own system we use (We will train you for this part).
• Pre-Billing.
• Act as a direct link between family, the home care agency, and the insurance case manager.
• Communicate with patients and aides.
• Ability to staff cases.
• Problem solver / Good people skills.
• Communicate with MLTC to authorize services, adjust care hours, report changes in patient health, and updating care plans.

Qualifications

• Experience in healthcare administration, nursing, social work, or a related field*preferred*.
• Experience in home care, healthcare coordination, case management, or patient services a must.
• Knowledge of home care regulations and documentation requirements.
• Strong organizational, problem-solving, and multitasking abilities.
• Excellent communication and interpersonal skills.
• Proficiency with electronic health records (EHR) and Microsoft Office applications.
• Ability to work independently and collaboratively in a fast-paced environment.
