Responsibilities

• Review provider schedules and patient charts to coordinate care.
• Handle non-appointment calls and route to appropriate party.
• Relay information between patients and medical staff; gather patient data as needed.
• Support medication refill processes for patients.
• Use registry data to inform preventive care and follow-up.
• Track and enter patient data for follow-up and reporting.
• Coordinate with case management for complex medical/psychosocial cases.
• Assist in chronic disease management programs (diabetes, hypertension, etc.).
• Coordinate with pharmacies and insurers to ensure information flow.
