Job summary

Virginia Mason Franciscan Health is seeking a Care Coordination Assistant to act as a central hub for patient care processes across all ages. The role combines frontline reception duties with clinical coordination, ensuring safe, therapeutic care and smooth information flow for the care team.

You will collaborate with the Care Coordination team, coordinate post-acute referrals, and maintain communications with patients, families, and clinicians to progress care and prevent readmissions.

Qualifications

• Associates degree in Business/Health-Related field required at hire.
• High School Diploma or GED required at hire.

Responsibilities

• Collaborates with and supports the Care Coordination team (RNCC, Social Worker).
• Manage timely post-acute care referrals to assist with length of stay management and denials mitigation.
• Communicate with interdisciplinary teams, patients, and families to facilitate care progression.
• Ensure post-acute follow-up appointments to assist with readmission prevention and continuity of care.
• Deliver required patient notifications and letters and obtain signatures for regulatory compliance.
• Manage transportation referrals as requested.
