We are seeking a Bilingual Blended Case Management Supervisor to lead and support our case management team. In this role, you will supervise Targeted Case Managers, provide guidance on service delivery, ensure compliance with program standards, and oversee the coordination of care for adults in the community. You will play a key role in fostering a collaborative team environment and ensuring high‑quality, client‑centered services.
