This is a full-time position in the Philadelphia area (19134) requiring bilingual Spanish‑English proficiency. Salary is $55,702 per year, with Monday through Friday 8:30 AM to 5:00 PM scheduling, and on-site supervision of blended case managers

Qualifications

• Master's degree in social work, psychology, or related field is required.
• Bilingual English/Spanish with strong reading and writing skills.
• Three years of mental health direct care experience, supervisory experience preferred.

Responsibilities

• Ensure DBH Practice Guidelines are applied in TCM duties.
• Provide on-site supervision to Blended Case Managers and hold team meetings.
• Deliver weekly supervision and document sessions with review of charts.
• Monitor 30-day contact board to ensure timely contacts.
• Ensure Pay-4-Performance criteria with compliant documentation.
• Monitor staff productivity via reports from the TCM director.

Job Summary

We are seeking a Bilingual Blended Case Management Supervisor to lead and support our case management team. In this role, you will supervise Targeted Case Managers, provide guidance on service delivery, ensure compliance with program standards, and oversee the coordination of care for adults in the community. You will play a key role in fostering a collaborative team environment and ensuring high‑quality, client‑centered services.

Key Responsibilities

• Ensure that the DBH Practice Guidelines are applied within the general operations of the TCM duties.
• Provide daily on‑site supervision to teams of Blended Case Managers, including team meetings to discuss daily contacts and support needs.
• Deliver weekly individual supervision to each assigned staff, documenting sessions and conducting chart reviews using the updated Utilization Review sheet.
• Monitor the 30‑day contact board and ensure contacts are completed before infractions occur.
• Ensure the Pay‑4‑Performance criteria are met, including timely, legible, signed, dated documentation that follows the DAP format.
• Monitor staff productivity using reports provided through the TCM director.

Requirements

• Masters‑level degree in social work, psychology, rehabilitation, activity therapies, counseling or education and three years of mental health direct care experience, one of which includes supervisory experience preferred.
• Knowledge of and ability to put into practice the Blended Case Management Standards of Care.
• On‑call responsibility and completion of BHTEN 10‑week case management orientation, as well as a valid driver’s license, is required.
• Bilingual proficiency in English and Spanish, with strong reading and writing skills.
• Current Driver’s License; satisfactory driving record; eligible to operate program vehicles.
