Job summary

FHCHS of PR, Inc. is seeking a Case Manager to support the Behavioral Health Contact Center. You will respond to individuals in need of services, perform needs assessments, and facilitate referrals to appropriate care levels.

Work involves coordinating with licensed clinicians and documenting services with HIPAA-compliant records. Responsibilities include handling emergency calls with clinicians, educating members about benefits, and ensuring timely access to services.

Qualifications

• Bachelor’s in social work or psychology.
• Microsoft Office knowledge.
• Fully bilingual preferred.
• Customer service experience.
• Excellent interpersonal skills.
• Available to work rotating shifts.

Responsibilities

• Answer incoming calls within established timeframes and identify oneself.
• Conduct needs assessments and facilitate referrals based on presenting concerns.
• Coordinate with licensed clinicians for emergency calls and urgent access to help.
• Guide members to appropriate levels of care and collaborate with emergency services when needed.
• Verify eligibility and educate members on program offerings and benefits.
• Document calls and service requests following HIPAA and internal policies.
• Maintain confidentiality and HIPAA-compliant documentation.
• Perform outbound calls and follow-up on referrals as assigned.
