Responsibilities

• Assist prospective clients and families through admissions with patience and clear guidance.
• Gather and document patient information, insurance details, and medical history for admissions.
• Schedule initial and ongoing home health visits coordinating with nursing and therapy teams.
• Communicate with clients, families, referral sources, and staff to ensure timely intake and scheduling.
• Confirm appointments, document changes, and update scheduling systems.
• Maintain organized records of admissions and scheduling activities.
• Collaborate with clinical staff to ensure care continuity.
• Respond to inquiries with professional customer service.
• Support compliance with patient confidentiality and data security regulations.
• Perform other administrative duties as assigned.
