Job summary

Baptist Health is seeking a Physical Therapist for the BHMG Outpatient PT Charlestown location in Indiana. The role focuses on evaluating patients' outpatient care needs and implementing an appropriate plan using physician input, community resources, and the patient's support system.

The PT will provide skilled therapy procedures, educate patients and families, document care in electronic records, and work a 40-hour week with occasional shifts to accommodate patients.

Qualifications

• Current active or current temporary Indiana Physical Therapy License.
• BLS certification within 90 days of hire.
• Ability to work 40 hours per week with occasional late or early shifts to accommodate patients.

Responsibilities

• Evaluates the patient's comprehensive outpatient health care needs and implements a plan using physician, community resources and patient support systems.
• Provides skilled physical therapy procedures/assessments and educates patients and their families in the plan of care.

Job Summary
