Job summary

Baptist Health is seeking a Full Time Physical Therapist for outpatient services. The PT will evaluate patients in the Charlestown outpatient clinic, develop and implement plans of care, and coordinate with physicians and resources to support recovery.

The role requires valid Indiana PT licensure and BLS within 90 days of hire. This position emphasizes high-quality patient care, critical thinking, and effective documentation using electronic records.

Qualifications

• Current active or current temporary Indiana Physical Therapy License
• BLS certification within 90 days of hire
• Ability to work 40 hours per week with occasional late or early shifts to accommodate patients

Responsibilities

• Evaluate the patient’s health care needs in the outpatient clinic and develop a plan of care.
• Provide skilled physical therapy procedures and assessments; instruct patients and families on the plan of care.
• Document treatment and care using electronic records.
