Job summary

Baptist Health Physical Therapy, part of Baptist Health Medical Group, is seeking a dedicated PT for outpatient care in Indiana. You will assess patients, develop individualized plans, and coordinate with physicians and community resources to support recovery and health maintenance.

We value critical thinking, compassionate patient care, and teamwork. The role requires documenting care electronically and maintaining licensure and BLS certification.

Qualifications

• Current active Indiana Physical Therapy license.
• BLS certification within 90 days of hire.

Responsibilities

• The PT evaluates the patient’s comprehensive health care needs in the outpatient clinic and implements an appropriate plan according to those needs utilizing the physician, community resources and the patient’s support system.
• Provides skilled physical therapy procedures/assessments and instructs patients and their families in the plan of care.
• Utilize electronic records to document treatment & care.

Requirements:

• Current active or current temporary Indiana Physical Therapy License
• BLS certification within 90 days of hire
• Ability to work 40 hours per week with occasional late or early shifts to accommodate patients
