Job summary

Heart to Heart Hospice is seeking a Bereavement Coordinator to assess bereavement needs and coordinate support for hospice families. You will work with the interdisciplinary team to ensure timely, compassionate bereavement interventions and referrals to community resources.

Ideal candidates have experience in bereavement counseling, strong communication skills, and knowledge of eligibility standards. This role includes training volunteers, documenting care, and ensuring regulatory compliance.

Qualifications

• High School graduate; counseling degree/license preferred
• Experience in bereavement and grief counseling, preferably in hospice/palliative care
• Working knowledge of community resources
• Strong communication, interpersonal, negotiation, and public relations skills
• EMR software experience is a plus
• Current state driver’s license, reliable transportation, and current auto liability insurance

Responsibilities

• Assess hospice family to identify bereavement needs and refer to community resources
• Maintain knowledge of eligibility standards (public and private)
• Collaborate with interdisciplinary team to coordinate bereavement care plan and interventions
• Provide ongoing bereavement support to hospice families with documentation
• Participate in QAPI
• Train and supervise volunteers supporting the bereavement program
• Timely submission of all documentation
• Perform other duties as required
• Ensure compliance with laws, Medicare regulations, and personnel policies

What You Must Have:

• High School Graduate, Degree/License in counseling preferred
• Experience in bereavement and grief counseling, preferably in hospice and palliative care setting; grief support counseling
• Working knowledge of community resources
• Demonstrates good communication, interpersonal, negotiation, and public relations skills
• Experience with EMR software is a plus
