Job summary

Mosaic in Colorado is seeking Home Providers to welcome individuals with intellectual disabilities into their home and life. You will help create a safe, valued, and connected living environment, matching individuals based on interests and support needs.

Responsibilities include daily living support, medication assistance, community engagement, transportation, and documenting progress with Mosaic. Training and ongoing support are provided to help both providers and individuals thrive.

Qualifications

• Must be at least 18 years of age
• Colorado resident due to state funding restrictions
• Own or rent own home (house, apartment, condo, townhouse)
• Active homeowner's or renter's insurance
• Reliable transportation, valid driver's license, current vehicle insurance
• Internet access and basic computer skills
• Ability to complete required screenings, background checks, and training

Responsibilities

• Support daily living skills such as cooking, budgeting, cleaning, and decision-making.
• Assist with medication administration and medical appointments.
• Help someone build connections, participate in their community, and foster a sense of belonging.
• Provide transportation and support with daily activities as needed.
• Maintain documentation and communicate with the Mosaic support team.
• Help someone build confidence, develop independence, and pursue personal goals.
