We are looking to hire a compassionate Behavioral Health & Wellness Specialist. This role is responsible for providing access to community resources, daily living skills training and support surrounding wellness for those living in our supportive housing programs.

POSITION RESPONSIBILITIES

  • Complete comprehensive needs assessments to guide individualized care.
  • Help individuals connect with community services such as case management, clinical care, day programs, and vocational opportunities.
  • Conduct outreach to local agencies and wellness providers to support individual needs.
  • Support recovery planning using wellness and recovery-based approaches.
  • Meet regularly with individuals to build skills and monitor progress.
  • Teach daily living skills like money management, medication monitoring, and community involvement.
  • Assist with medication management and offer support related to medication use.
  • Provide life skills coaching and transportation to support individuals' health and wellness goals.
  • Maintain regular communication with program staff to coordinate care plans.
  • Attend supervision sessions and team meetings as required.
  • Other duties as assigned.

POSITION REQUIREMENTS

  • Bachelor's Degree in Related Field preferred
  • High School diploma or equivalent and relevant experience required
  • CASAC certification preferred
  • Experience with people coping with mental illness, MICA
  • Valid NYS Driver's License in good standing and access to reliable personal vehicle
  • Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehicle

POSITION RELATIONSHIPS

  • Reports to: Community Services Coordinator
  • Supervises: None

*Training will be completed Monday-Friday, 8:30am - 4:30pm*

SCHEDULE: Tuesday - Saturday, 8:30am - 4:30pm

PAY RATE: $21.00 hourly

LOCATION: Main Office, Islip Terrace, NY
