Qualifications

  • High school diploma or GED required.
  • Current Peer Support Specialist, Peer Recovery Support Specialist, or Recovery Support Specialist certification.
  • Familiar with behavioral health community resources.
  • Arizona Fingerprint Clearance Card may be required at hire.

Responsibilities

  • Coordinate transition out of care, referrals, transportation, and follow-up.
  • Model recovery through coaching to support client skills development.
  • Transfer patient information and conduct follow-up with patients and providers.
  • Attend team meetings and clinical staffing as needed.
  • Complete progress notes and timely reports per policy and funding guidelines.
  • May lead support groups and activities to improve life and social skills.
  • Identify unmet needs and connect patients to community resources.
  • Complete required training and participate in supervision.
  • Collaborate with care teams and external partners to support patients.
