• Arizona Fingerprint Clearance Card at time of hire for Banner Behavioral Health Hospital, BTMC Behavioral, and BUMG/BUMCT/BUMCS facilities.

Responsibilities

• Coordinate transition from hospital care to post-acute settings or community services as directed by care coordination team.
• Provide coaching and role modeling to support patient recovery and skills development.
• Transfer of patient information between acute and post-acute care entities; follow up with patients and providers as needed.
