Job summary

Metrowest Medical Center - Consolidated is seeking a qualified social work professional to assess patients and develop individualized Master Treatment Plans, with ongoing reviews. You will conduct mini-mental status exams, psychosocial assessments, and facilitate group and individual therapy, documenting in a compliant, timely manner.

We value licensed or registered professionals, CPR certification, and at least 2 years of related experience.

Qualifications

• Master's degree in Social Work is required.
• Minimum 2 years of related experience.
• Currently licensed, certified, or registered to practice as required by law; CPR is required.

Responsibilities

• Designs a unique Master Treatment Plan for each patient based on diagnosis and issues identified in the assessments.
• Reviews and updates treatment plans every 30 days.
• Performs a mini mental status exam and psychosocial assessment on all new patients.
• Provides group and individual therapy and prepares timely documentation in compliance with company, state, and federal guidelines.
• Performs related duties as required
