Job summary

Responsibilities

• Designs a unique Master Treatment Plan for each patient based on diagnosis and issues identified in the assessments.
• Reviews and updates treatment plans every 30 days.
• Performs a mini mental status exam and psychosocial assessment on all new patients.
• Provides group and individual therapy and prepares timely documentation in compliance with guidelines.
• Contacts patients who are absent for 3+ days to determine plans for return to the program.
