Job description

CPC's PACT team is seeking a dedicated and compassionate Behavioral Health Clinician to join our team. The ideal candidate will be passionate about providing high-quality mental health services to individuals in need.

Location: Redbank, NJ

About Our Role

As a Behavioral Health Clinician I for PACT (Program of Assertive Community Treatment), you will serve people in the community as part of a multi-disciplinary team of professionals. The PACT team works together to help adults with mental illness learn the tools to become and remain stable, healthy, and successful, while living in the community. This is done through individual and group treatment modalities. The Behavioral Health Clinician provides assessments and direct services for people with serious and persistent mental illness and persons with co-occurring substance use issues enrolled in the PACT Program.

This position is 100% community based.

Responsibilities:

  • Work cooperatively and in coordination with PACT team members, consumers, for their families and other service providers.
  • Complete culturally sensitive biopsychosocial assessments.
  • Provide individual, group, and family interventions as warranted, based on development of individual recovery plan.
  • Assess psychoeducational learning needs of consumers and their families, as applicable.
  • Assist consumers with daily living tasks, including shopping, cooking, housekeeping, etc.
  • Respond to emergency situations
  • Help consumers clarify choices in relation to housing, employment, school, use of leisure time, etc.
  • Help consumers find and maintain employment, housing, benefits, etc.
  • Act as advocate for consumers, which may involve frequent contact with employers, landlords, local shopkeepers, and others.
  • Adhere to all Agency policies and procedures, including attendance at all required agency training

Requirements:

  • Master's Degree and LSW or LAC
  • Valid New Jersey driver's license
  • 2 years post-bachelor's level experience working with individuals with serious and persistent mental illness
