Job summary

Central City Health, Inc. seeks a Director, Case Management and Social Health to provide leadership and oversight for an integrated case management workforce spanning behavioral health, housing, and social support services.

The role leads care coordination across programs to promote continuity of care, improve outcomes, and address social determinants of health in the community. Strong leadership and change-management experience are essential.

Qualifications

• Master's degree in social work, public health, or related field.
• Minimum 5 years leadership experience spanning case management, care coordination, or social services.
• Demonstrated experience managing organizational change.
• Familiarity with IPS Supported Employment fidelity, HUD housing program requirements, and 42 CFR Part 2 confidentiality boundaries for any SUD-adjacent case management work.

The Director leads care coordination and service navigation across programs to promote continuity of care, reduce fragmented service delivery, and ensure individuals with complex needs receive coordinated, person-centered support. This role oversees case management operations, workforce performance, caseload management, and program fidelity while collaborating with clinical, housing, and organizational leadership to address social determinants of health and improve patient outcomes.

Education/Experience

• Master's degree in social work, public health, or related field; Michigan LMSW or equivalent licensure preferred (though this role's clinical supervision authority sits with Clinical Supervision & Practice, not this seat).
• Minimum 5 years leadership experience spanning case management, care coordination, or social services, ideally across both behavioral health and housing/SDOH contexts.
• Demonstrated experience managing organizational change.
• Familiarity with IPS Supported Employment fidelity, HUD housing program requirements, and 42 CFR Part 2 confidentiality boundaries for any SUD-adjacent case management work.
