Job summary

Responsibilities

• Fulfill patient care responsibilities and organize patient flow for timely visits.
• Maintain accurate documentation in the electronic health record.
• Assist in telephone triage and follow-up calls as directed by the provider or manager.
• Assist with patient health education and follow-up on care plans.
