Job summary

Under RN/LPN supervision, you will coordinate with physicians and families, lead therapeutic activities, and monitor patient wellbeing.

Qualifications

• Assesses and treats patients across age groups.
• Coordinates care with physicians, families, and team.
• Must have CPR/BLS and First Aid certifications within 30 days.

Responsibilities

• Assesses and treats children, adolescents, and adults seeking psychiatric/chemical dependency treatment.
• Communicates needs to physicians accurately and completely.
• Familiarizes self with the care plan and contributes insights.
• Collaborates with referral sources and families for care coordination.
• Provides follow-up actions as needed.
• Acts as initial contact for patients and families seeking intervention.
• Documents intake screening reports and patient observations.
• Performs vital sign checks and basic cognitive assessments.
• Leads patients in therapeutic and recreational activities.
• Monitors wellbeing and reports issues to medical staff.
• Assists with activities of daily living as needed.
