Job summary

Children’s Home Society of Florida (CHS) is seeking a Targeted Case Manager to assess needs, coordinate services, and advocate for vulnerable children and families across homes, schools, and communities.

You will conduct comprehensive assessments, connect clients to resources, document outcomes, and participate in care planning to move families from crisis to resilience. Requires a relevant degree and AHCA training within 3 months.

Qualifications

• Bachelor's Degree required in Human Services/Social Work or related field; other degrees may qualify with relevant experience.
• Minimum 1 year working with severely emotionally disturbed children (or 3 years if degree is outside Human Services).
• Florida Driver's License with access to a reliable, insured vehicle.
• Must be able to work Mon-Fri, 8-5 with flexibility for after-hours when needed.
• AHCA-approved Targeted Case Management certification training within three months of hire.

Responsibilities

• Conduct comprehensive assessments to identify client and family needs and develop individualized service plans.
• Connect children and families to critical community resources and treatment services.
• Advocate for clients across schools, healthcare systems, behavioral health providers, and community organizations.
• Provide in-home support and guidance to families.
• Document progress and outcomes through the EHR system.
• Participate in family service planning, case reviews, and community outreach.
