Qualifications

• Master’s degree in counseling psychology, clinical psychology, applied psychology, social work, marriage & family therapy, mental health, or equivalent.
• Possess current CPR certification or willingness to obtain within 6 months of employment by LifeCircles.
• Experience providing behavioral health services with older adults or individuals with serious mental illness in medical settings.
• Experience with substance use treatment and caregiver training.
• Knowledge of evidence-based practice and integrated care approaches.
• Ability to identify differential diagnoses and inform treatment planning.

Responsibilities

• Collaborate with interdisciplinary team to provide comprehensive care and align medical and behavioral health strategies.
• Train staff on mental health care approaches and best practices.
• Coach care partners to enhance their skills in supporting participants’ behavioral health needs.
• Conduct bio-psychosocial assessments to evaluate holistic needs.
• Perform differential diagnoses to inform personalized care plans.
• Provide behavioral health consultation and create tailored treatment plans.
• Develop strategies to support autonomy and community living.
• Deliver direct behavioral health interventions through individual and group therapy.
• Provide services across day centers, homes, acute and long-term care settings.
• Lead program development and quality improvement initiatives.
