Qualifications

• Masters degree in Social Work, Counseling or Psychology.
• Current Michigan certification.
• Current BLS for the Healthcare Provider Certification completed by 60 days after hire.
• Independent Practice Certification in area of expertise (LMSW, Substance Abuse, CAC, LPC).
• 2 years experience working with adult mentally ill individuals in an acute care setting preferred.
• Computer literate.

Responsibilities

• Completes timely and accurate initial assessment including psychosocial history, psychiatric illness, family relationships, stressors and support systems.
• Includes patient, family/significant others, and treatment team in development of patient-centered care plan.
• Provides individual, family and group therapy as indicated and coordinates discharge plans including aftercare.
• Documents interactions and progress in the electronic chart; completes necessary forms.
• Attends and participates in team meetings; maintains licensure and CEU requirements.
