Job summary

Trinity Health IHA Medical Group is seeking a Behavioral Health Specialist to join our collaborative care team. You will support patients with behavioral health concerns through assessment, care coordination, brief evidence-based interventions, education, and monitoring to improve outcomes and patient experience.

This role emphasizes coordination with primary care providers, psychiatrists, and other clinicians, with a focus on evidence-based approaches and telehealth for accessible care.

Qualifications

• Master’s degree in social work or related behavioral health field (required).
• Valid Michigan clinical licensure (LMSW, LPC, LMFT, or similar).
• Experience with evidence-based interventions (CBT, PST, or Behavioral Activation) preferred.

Responsibilities

• Support behavioral health care coordination with primary care providers and specialists.
• Screen and assess patients for common mental health and substance use disorders.
• Provide patient education on behavioral health conditions and treatment options.
• Monitor patients for changes in symptoms, treatment response, and medication side effects (in-person and telehealth).
• Promote treatment adherence and identify barriers to psychotropic medication management.
• Deliver brief evidence-based interventions such as Behavioral Activation.
• Track patient outcomes and document clinical encounters.
