Trinity Health IHA Medical Group in Southeast Michigan is seeking a compassionate Behavioral Health Specialist to join our Behavioral Health Collaborative Care team. You will support patients with behavioral health concerns through assessment, care coordination, brief evidence-based interventions, patient education, and ongoing symptom monitoring.

You will coordinate with primary care providers and psychiatric consultants, screen for disorders, monitor for side effects, and track outcomes.

Formación

• Master's degree in a behavioral health field and Michigan licensure.
• Valid Michigan clinical licensure (LMSW, LPC, LMFT, or similar).
• Experience with CBT or Behavioral Activation preferred.

Responsabilidades

• Coordinate behavioral health care with primary care providers and specialists.
• Screen and assess for mental health and substance use disorders.
• Educate patients on behavioral health conditions and treatment options.
• Monitor symptoms and treatment response via in-person and telehealth.
• Support psychotropic medication management and adherence.
• Deliver brief evidence-based interventions (e.g., Behavioral Activation).
• Track outcomes and document encounters.
