Job summary

Oak Street Health is seeking a Behavioral Health Specialist (requires LCSW, LPC, or LMFT) to provide care management and short-term evidence-based psychotherapy across primary care clinics. Qualifications

• Master’s degree in mental health counseling/healing arts from an accredited school.
• Highest clinical level license (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral, etc.).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and refer to psychiatric consultant or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant, including warm handoffs for telehealth.
• Maintain relationships with primary care teams and communicate changes in care plans.
• Document patient encounters and sessions in the EMR.
• Participate in regional and organizational efforts to advance the program.
• Possibly cover multiple clinics depending on needs.
• Perform other duties as assigned.

Education
