Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy in our primary care network. The role includes coordinating with care teams, making referrals to psychiatric resources, and delivering evidence-based counseling.

Candidates must hold a Master’s degree in a mental health field and the highest clinical license (LCSW/LCPC/LMFT) with US work authorization. A remote work option may be possible based on program metrics and performance.

Qualifications

• Master's degree in mental health field required.
• Highest clinical license (LCSW/LCPC/LMFT) or local equivalent.
• Experience screening for mental health/substance use disorders.
• Experience with assessment, treatment planning, and referrals.

Responsibilities

• Collaborate with primary care teams to refer patients to the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and coordinate with psychiatric consultants or external resources.
• Provide short-term counseling using evidence-based treatments.
• Document patient encounters thoroughly in the EMR.
• Support regional/organizational efforts to advance the program.
