Qualifications

• Bachelor's degree in psychology, human services or related field.
• Valid driver’s license required if assigned to drive company vehicle; proof of insurance if driving personal vehicle.
• CPR required within 60 days; CPI required within 90 days.
• 1 year of experience is preferred, relevant experience considered.
• Must have good verbal and written communication skills.
• Must be able to lift 50 pounds, assist with 100 pounds.

Provides assistance to the multidisciplinary teamrelated to placement, discharge planning, treatment, linkage to community resources and crisis intervention. Coordinates with team to achieve quality and cost effective patient outcomes.

Education

Licensure/Certification
