Saint Vincent Hospital in Worcester is seeking a Social Worker to coordinate care across a patient’s transition plan, promoting timely throughput and safe discharge. The role emphasizes complex psycho-social assessments, family communication, and collaboration with a multidisciplinary team.

Responsibilities include leading care conferences, ensuring appropriate resource use, and aligning with state and federal regulations.

Qualifications

• Bachelor's degree in Social Work required.
• Must be licensed or license-eligible to practice as LICSW, LCSW, or LMHC per state requirements.
• 2 years of acute hospital experience preferred.

Responsabilités

• Complex psycho-social transition planning, assessment, reassessment and intervention.
• Assist with adoptions, abuse and neglect cases including assessment, intervention and referral to agencies.
• Care coordination or oversight of the transition plan implementation.
• Lead or facilitate multidisciplinary patient care conferences including Complex Case Review.
• Communicate plan of care with patients and families; perform other duties as assigned.
