Job summary

Qualifications

• Previous psychiatric or behavioral health nursing experience is preferred.
• Experience working with youth and families in behavioral health, healthcare, or community-based settings is preferred.
• Experience working in community-based or off-site settings is preferred.
• Must be comfortable and willing to provide services in community settings as required by the position.
• Strong communication, organizational, clinical assessment, and care coordination skills are preferred.

Responsibilities

• Conduct comprehensive nursing assessments that consider the relationship between physical and mental health.
• Identify medical concerns, risk factors, barriers to care, and other healthcare needs; coordinate services and follow-up with providers.
• Develop and implement individualized service plans with clients, youth, and families; set measurable goals.
• Transcribe, administer, monitor, evaluate, and document prescribed medications; monitor effects.
• Provide direct nursing care, education, and crisis intervention; support symptom management and independence.
• Maintain accurate documentation of all services provided in Avatar.
