Qualifications

• Associate Degree in Nursing required.
• New York State RN license required.
• Valid driver's license, vehicle, and verifiable insurance.
• Case Management Certification preferred.

Responsibilities

• Conduct assessments, care planning, and ongoing care management across hospital, sub-acute, long-term care, and home settings.
• Coordinate services with members, caregivers, physicians, and interdisciplinary teams to address healthcare needs and goals.
• Perform home visits to evaluate living conditions, functional status, cognitive needs, and community support requirements.
• Develop and maintain comprehensive Patient-Centered Service Plans while monitoring eligibility and service utilization.
• Support discharge planning, care transitions, grievance resolution, and documentation activities.
