Additional Information About the Role

• Shift information: Monday – Friday, 8 AM – 5 PM; no holidays, no weekends
• Client Interaction: This is an educational and teaching role where you work with clients on health literacy, assist them in scheduling primary care physician appointments, and educate them about blood sugar health. The role also involves performing finger sticks to check A1C levels.
• Case Load Size: 200 clients shared among 15 RN Case Managers, with clients seen once a year for verbal assessments
• Location: 3309 South Kingshighway, St. Louis City
• Ideal Skill Set: RN license with any RN experience; Health Education or Home Health RN experience is a plus

Overview

BJC Behavioral Health is a community health center providing behavioral health services to over 8,000 seriously mentally ill adults and emotionally disturbed children across several counties in Missouri. As an Administrative Agent of the Missouri Department of Mental Health (DMH), BJC serves as a primary entry point and gatekeeper to public mental health services.

Preferred Qualifications

Role Purpose: Develops a wellness culture integrating behavioral and medical healthcare, alerting providers to co-morbid conditions, coordinating enrollment in health programs, and monitoring effective health strategies in treatment plans.

Responsibilities:

1. Alert healthcare providers to co-morbid medical and behavioral conditions, coordinate client enrollment in health programs, and monitor health strategies in treatment plans.
2. Track, monitor, and distribute reports.
3. Consult on clients' health conditions and provide education training.
4. Assist in developing and enhancing wellness initiatives.
5. Support referrals and discharge planning.

Minimum Requirements:

Education: Associate's Degree in Nursing

Experience: 2-5 years

Supervisor Experience: None required

Licenses & Certifications: Valid Driver's License, RN license

Preferred Requirements:

Education: Bachelor's Degree in Nursing

Experience: 5-10 years
