Job summary

Kickapoo Tribe in Kansas is seeking an Integrated Resource Coordinator to manage a caseload of clients with long-term mental illness or substance use issues. The role involves assessing client needs, developing and reviewing service plans, and coordinating with community resources to meet client needs.

The position requires developing treatment/service plans, maintaining detailed records, and collaborating with other departments.

Qualifications

Bachelor's degree in relevant human services field or equivalent experience.
Experience or knowledge in serious mental health diagnoses or substance use disorders.
Strong ability to maintain detailed records and develop professional relationships.

Responsibilities

Screen new service requests.
Participate in interdisciplinary staff meetings.
Complete case management assessments for all clients entering caseload.
Develop and implement treatment/service plans with clients and service providers.
Document all service contacts in a timely manner.
Participate in interagency planning and service coordination activities.
Meet regularly with the Project Director/Integrated Care Manager for review and growth.
Collaborate with and provide clinical consultation within the department.
Travel for local and out-of-state conferences and trainings for professional development.
