Job summary

Swope Health seeks a Community Behavioral Health Case Manager to connect community members with essential services and support their recovery. You will work with a dedicated team to assess needs, coordinate treatment plans, and facilitate access to mental health resources in the Kansas City area.

Responsibilities include assisting with housing, financial aid, and transportation; coordinating with services; and documenting for reimbursement.

Qualifications

• Bachelor's degree in a human services field or equivalent combination of education and experience.
• Two years of higher education with two years of related psychiatric/substance abuse experience, or any four-year degree with two years of related experience.
• Four years of equivalent experience in psychiatric, substance abuse, or developmental disabilities services.
• One to three years experience with SMPI/SED clients preferred.
• Must have a valid driver's license, transportation, and car insurance; ability to drive extensively.

Responsibilities

• Assess and monitor an individual's adjustment to community living.
• Monitor participation and progress in treatment programs per the treatment plan.
• Develop or revise a Person Centered Individualized Treatment Plan.
• Help individuals access mental health services and accompany to appointments.
• Assist with accessing public services, including housing, food, and shelter needs.
• Link clients to community resources for ongoing social and vocational supports.
• Support discharge planning and follow-up after hospitalization.
• Provide daily living skills coaching and independence training.
• Involve families and guardians in planning and support processes.
• Travel extensively across the Kansas City metro to visit clients.
