Job summary

Kanza Mental Health And Guidance Center Inc is seeking a Care Coordinator for its Outpatient Department in the United States. You will manage intake, referrals, and ongoing care coordination, working with a multidisciplinary team to enhance client engagement and outcomes.

Qualifications

• High School Diploma or equivalent required.
• Associate’s degree preferred but not required.
• At least two years of applicable work experience that supports the skills and requirements of the position.
• Strong organizational, time management, communication and interpersonal skills.
• Excellent communication and interpersonal skills.
• Intermediate computer skills and proficiency required.
• Bilingual skills are a plus, but not required.

Responsibilities

• Provide warm and friendly customer service in person, by phone, and in writing to individuals and families receiving services.
• Understand and follow HIPAA and 42 CFR Privacy Laws for patients with mental health and substance use disorders to ensure confidentiality and privacy.
• Develop and maintain positive working relationships with care providers and community partners to ensure good coordination of care.
• Assist individuals and families with intake, paperwork, and referrals, including consent forms and releases of information.
• Document all contacts and activities in the electronic health record in a timely and accurate manner.
• Link individuals and families with resources and monitor services per the treatment plan.
• Complete surveys with individuals and manage data entry as required.
• Support applications for benefits such as KanCare Medicaid and transportation resources.
• Participate in team meetings and ongoing professional development.
