Community Health & Wellness Partners (CHWP) in Urbana, OH seeks a Behavioral Health Provider to join the behavioral health team. The provider will screen records, conduct assessments, set treatment goals and deliver interventions within a team-based primary care model.

Requires a Master’s degree in Social Work or related field, Ohio licensure, and experience in behavioral health. Full-time, on-site role with duties including documentation, referrals, and collaboration with PCPs and the BH team.

Qualificações

• Master’s degree in Social Work, Counseling, or related field.
• Valid Ohio drivers license and applicable professional licensure (LSW/LISW/LISW-S, LPC, LPCC or LPCC-S).
• Experience in behavioral health and substance use practices is preferred.

Responsabilidades

• Screen medical records and conduct interviews and assessments.
• Set treatment goals and provide interventions for patients.
• Provide psychoeducation and collaborate with PCPs and BH team.
• Maintain confidentiality and complete documentation within 7 days of service.
