Job summary

Qualifications

• A bachelor’s degree in counseling, social work or related field is required.
• Active license in Ohio as LSW or LPC is required.
• Master’s degree is preferred.
• Experience serving multi-need individuals and families is required.

Responsibilities

• Conduct biopsychosocial assessments under supervision.
• Engage individuals and families in home/community to identify strengths and goals.
• Provide therapeutic interventions within licensure and supervision.
• Develop trauma-informed, strengths-based treatment plans.
• Coordinate with internal and external teams to ensure continuity of care.
• Maintain timely, accurate, compliant documentation.
• Deliver care in home and community-based settings.
• Travel within designated service area.
