Job description

Responsibilities

Monday through Friday position with no on-call or weekend hours required.

InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities, providing customized healthcare, social engagement, and exercise to help older adults stay independent and in their homes. Participants receive most of their healthcare and social support at an InnovAge center in their community, with a dedicated team of healthcare experts and personalized care plans.

Our Behavioral Healthcare Provider (BHP) plays a key role in developing and delivering personalized, team-based behavioral health care. Responsibilities include conducting initial assessments, providing clinical interventions, making referrals, and offering treatment recommendations. Physical presence at an InnovAge center is essential, along with the ability to travel to meet participants outside of the center as needed.

You will
  • Navigate ambiguous situations confidently
  • Multitask, remain flexible, and adapt to changing environments
  • Interpersonally adapt to meet clinical needs of participants and the Interdisciplinary Team (IDT), leveraging personal strengths
  • Utilize knowledge of Motivational Interviewing, Solution-Focused Brief Therapy, Dialectical Behavioral Therapy, Acceptance and Commitment Therapy, and Cognitive Behavioral Therapy
  • Be receptive to feedback and reflect productively
Clinical Care – 70%
  • Complete mental health assessments and document in the EMR, including diagnosis, recommendations, care planning, and regulatory requirements
  • Provide solution-focused interventions (monitoring, therapeutic, or crisis) to participants and caregivers
  • Consult with team members regarding clinical findings, interventions, and case conferences
  • Collaborate with IDT and external providers to support participants at risk of inpatient psychiatric hospitalization, including referrals and coordination
  • Monitor response to psychotropic medications and coordinate with psychiatric consultants for continuity of care
Administration – 30%
  • Educate the IDT on managing and resolving behavioral complexities or placements
  • Liaise with external psychiatric providers to ensure proper referrals, follow-up, and care continuity
  • Participate in IDT meetings, conferences, committees, and events as requested
Minimum Requirements
  • Master’s degree in behavioral health, social work, or psychology
  • At least 1 year of experience in an integrated care setting
  • Licensed Mental Health Professional, such as:
    • Clinical Social Worker (LCSW)
    • Licensed Psychologist (PsyD or PhD)
    • Licensed Marriage and Family Therapist (LMFT)
    • Licensed Professional Counselor (LPC)
  • Current CPR certification or ability to obtain it within 30 days of employment
Preferred Qualifications
  • Experience working with geropsychiatric populations
  • Training or experience with group and family interventions
  • Skills in conflict management, mediation, de-escalation, and crisis mitigation
  • Knowledge of chronic disease management and psychosocial implications
  • Bilingual abilities
  • Doctoral degree in clinical psychology or experience in integrated primary care
  • Licensure or certification in substance use treatment
