Responsibilities

• Evaluates and treats referred patients using assessment tools and documents findings.
• Develops treatment plans and goals with the interdisciplinary team; provides patient education.
• Plans group treatment sessions and coordinates with staff to monitor progress.
• Ensures timely and accurate documentation in medical records; maintains equipment and supplies.
