Qualifications

Responsibilities

• Engage with patients in collaborative care management of behavioral health conditions.
• Screen for common mental health and substance use disorders; educate on treatment options.
• Support psychotropic medication management and monitor treatment adherence and outcomes.
• Conduct outreach for patient engagement and follow-up care.
• Provide brief behavioral interventions using evidence-based techniques.
• Develop relapse prevention self-management plans with patients.
• Monitor progress and document in EHR; track outcomes in a registry.
• Collaborate with psychiatry and primary care teams; coordinate referrals and social services.
