Job summary

UVA Health is seeking a full-time Behavioral Health Navigator (MSW) to support patients at UVA Health Primary Care Culpeper and UVA Health Pediatrics Culpeper.

The role is in-person with a 50/50 split between Culpeper clinics, collaborating with care teams to support Family Medicine and Pediatric patients and their families. You will help implement the Collaborative Care Model and coordinate care for patients in evidence-based, protocol-driven services.

Qualifications

• MSW from a CSWE-accredited program.
• BLS certification within 90 days of hire.
• Experience with screening for mental health/substance use disorders.
• Ability to work in a fast-paced primary care setting as part of an interdisciplinary team.

Responsibilities

• Engage with patients enrolled in collaborative care management of behavioral health conditions.
• Screen patients for mental health and substance use disorders.
• Provide patient education about disorders and treatment options.
• Support medication management and adherence monitoring.
• Conduct outreach for patient engagement and follow-up care.
• Provide brief behavioral interventions using evidence-based techniques.
