Job summary

Bergen New Bridge Medical Center is seeking a Licensed Practical Nurse in Behavioral Health Services to join our patient-centered team in Paramus, NJ. The role focuses on direct patient care under RN supervision, administering medications, and contributing to individualized plans of care.

Qualifications

• Current NJ license as an LPN.
• Successful completion of Progressive Care Orientation Program required.
• American Heart Association Basic Life Support certification required.
• IV certification preferred.
• Clinical nursing skills appropriate for an LPN.
• Good oral and written communication skills.
• Speaks, reads and writes English to the extent required by the position.

Responsibilities

• Collect data from patient, family or significant others and record on Initial Data Base form.
• Perform twenty-four hour nursing assessment and provide data to RN for MDS inclusion.
• Contribute to the formulation and updating of plan of care and implement plan under RN direction.
• Administer medications according to hospital policies.
• Recognize changes in patient condition and notify charge nurse.
• Document pertinent information in patient’s medical record.
• Maintain infection control and safety in patient care.
