Job summary

The Link Access Center (LAC) at Klamath Basin Behavioral Health in Southern Oregon is seeking an LAC Specialist to join our multidisciplinary team. This full-time role supports adults experiencing mental health and substance use challenges, guiding admissions, stabilization, discharge planning, and connections to community resources.

You will work with nursing and behavioral health staff, document care, and help create a recovery-focused environment, while ensuring client safety and compliance

Qualifications

• Candidates must meet Oregon requirements for designation as a QMHA.
• Valid Oregon driver’s license and insurable driving record.
• Current CPR and First Aid certification, or ability to obtain upon hire.

Responsibilities

• Assist admissions and discharges and document processes.
• Coordinate referrals to services and supports.
• Monitor client status and document observations.
• Participate in treatment planning and interdisciplinary care discussions.
• Deliver interventions as per individual service plans and provide safety.
• Maintain client records within required timeframes.
• Collaborate with nursing and behavioral health staff and community partners.
• Assist with daily living activities and community resource coordination.
